New Patient Registration
We have an open list and welcome registration requests from patients living in or moving to the practice area.
If you live within our Courtyard Surgery Practice Catchment Area .pdf you may come into the surgery and ask for a registration pack alternatively you can download the necessary forms to register with the practice below.
To register at Courtyard Surgery the following documents are required.
- GMS1 Registration form
- New patient questionnaire
- Red book/record of immunisations for children under 5
You will also be asked for identification to prove your identity plus evidence of your address in Horsham. Please note we will photocopy these documents for our records.
Acceptable forms of identification are:
- Photo Driving license/passport/birth certificate
- Bank or building society statement in your name showing your Horsham address
- Utility bill in your name showing your Horsham address
- Lease/tenancy agreement/rent book
If you are unable to produce identification to prove your identity or your address in Horsham we may not be able to register you.
Please note you will need to bring your completed forms with identification to the surgery in order for us to complete your registration.
