Sick notes (also known as fit notes or Med3)

If you’ve been off sick for 7 days or less

If you’re off work sick for 7 days or less your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form when you return to work; this is called self-certification.

If you’ve been off sick for more than 7 days

If you have been off for more than 7 days and this is your first sick certificate being issued by the practice for this episode of sickness you will need to contact the surgery. The easiest way to request a sick note is via SystmConnect; under the 'Admin request' section click on 'Fit note', then 'Fit Note Request NEW' and fill in the form. We will be in touch as soon as we can.

 

If you are looking to extend a current sick note, you can submit your request via SystmConnect; again under the 'Admin request' section click on 'Fit note', then 'Fit Note Request EXISTING' and fill in the form.

 

You will need to ensure you give all the relevant information in support of your request, to ensure your sick note is correct. 

Please note we do not issue 'fit to work' notes.
