HOW TO REGISTER
When you come into the surgery to
register as a new patient you will
need to produce identification of
your residency in Horsham. If you
are unable to produce identification
of Horsham residency we will be
unable to register you with the
practice.
Acceptable forms of identification
are:

Photo Driving Licence
-
Bank or Building Society
Statement showing your Horsham
Address
-
Utility Bill showing your
Horsham Address
Our receptionist will view your
documents and then ask you to
complete and sign your Medical Card,
if you have one, or the receptionist
will give you a GMS1 registration
form to complete. Parents of
children under 16 should complete
and sign on their behalf. Hand your
card or form back to the
receptionist. If you have a
preference for a particular GP
please bring this to our attention
and we will endeavour to accommodate
your request.
When registering
with the practice, you will be given
a health questionnaire for each
member of the family, a urine
specimen container, and, if
appropriate, a child’s immunisation
sheet. Please complete and
bring these with you at your
earliest convenience.
