COMPLAINTS AND COMMENTS
If you have either a complaint or
concern about the service that you
have received from doctors or staff,
please contact Janet Yeo the
Practice Manager. We operate a
practice complaints procedure as
part of the NHS Complaints
Procedure.
CONFIDENTIALITY
All records on file or on computer
are strictly confidential. They will
not be disclosed to anybody unless
we have your written permission to
do so. We are registered under the
Data Protection Act (1998).
PALS (Patient Advisory Liaison
Services)
You could also contact PALS (Patient
Advisory Liaison Services) on
01293 600399 who offer an information,
advice and support service for
patients, carers and relatives (they
are a link to the NHS Complaints
Procedure).
PRACTICE CHARTER
A copy of the Practice Charter is
available on request from the
Practice Manager or reception.
SICKNESS CERTIFICATES
You do not require a doctor’s
sickness certificate for any illness
lasting seven working days or less. Your
employer may however require you to
complete a self-certification form
(SC2) which is available from your
employer or the DHSS. For any
illness lasting longer than seven
days you will need to see the doctor
for him/her to issue a sickness
certificate (F.med3) and for any
subsequent renewal of the
certificate.
In exceptional circumstances a
private sickness certificate may be
issued at a cost of £14.50.
VIOLENT PATIENTS
The practice supports the
government’s NHS zero tolerance zone
campaign. Violent patients will be
reported to the police and struck
off the GP’s list. GPs and staff
have the right to care for others
without fear of being attacked or
abused. |
